Evaluation of therapeutic efficacy in a case of destructive impulse control disorder and generalized anxiety through dialectical behavior therapy
Violent behaviors and impulses have been studied for decades, influenced by both biological and environmental factors. In Colombia, traditional parenting models tend to normalize maltreatment, which has led to a considerable increase in cases of domestic violence. This clinical case reflects such a situation, as the client presents symptoms associated with impulse control disorder and anxiety, possibly stemming from and maintained by experiences of violence in childhood. Recognizing these patterns of behavior is important, as domestic violence manifests in various forms and impacts the mental health of those involved.
The present case study, based on an A-B design, aims to evaluate the effectiveness of a protocol based on Dialectical Behavior Therapy (DBT) focused on the regulation and management of impulses associated with possible intermittent explosive disorder and generalized anxiety disorder. To assess the effectiveness of the treatment, measures such as the Percentage of Non-Overlapping Data (PND), the Percentage of Data Exceeding the Median (PEM), and the Non-Overlap of All Pairs (NAP) were employed, along with monitoring of the psychometric instruments BAI and STAXI.
The results indicate that the DBT-based intervention protocol was not completely effective in managing anger, although it did achieve a reduction in anxiety symptoms. Nevertheless, it will be necessary to continue addressing the persistent symptoms to ensure that all aspects of anxiety and impulse control are comprehensively managed.
