The integration of digital technologies has transformed the way clinical images and patients' sensitive information are acquired, stored, shared, and used in medical practice. Although these resources are essential for clinical care, medical education, and research, they have also increased challenges related to privacy, confidentiality, and respect for patient autonomy. The aim of this study was to review the historical, bioethical, and regulatory foundations that guide the responsible management of clinical images in scientific and digital settings. A narrative review of scientific publications and regulatory documents was conducted to examine the evolution of visual documentation in medicine, informed consent, anonymization, secure data storage, and the risks associated with the use of electronic devices, the Internet, and digital platforms. The review showed that clinical images constitute sensitive information whose use requires clear ethical, legal, and technical standards, as well as institutional procedures to ensure their protection. It is concluded that strengthening data protection policies, promoting best practices, obtaining appropriate informed consent, and maintaining continuous updates in response to technological advances are essential measures to safeguard patients' rights.
