Architectures of indigenous and intercultural health: a comparative analysis of SISPI (Colombia), SAFCI (Bolivia), and the first nations health authority - FNHA (Canadá)

Objective. To comparatively analyze the Indigenous and Intercultural Health System (SISPI) of Colombia, the Family, Community and Intercultural Health Model (SAFCI) of Bolivia, and the First Nations Health Authority (FNHA) of Canada, identifying their conceptual foundations, governance structures, and implementation challenges. Method. A qualitative, descriptive, and comparative study was conducted based on documentary review of academic literature, official regulations, and institutional documents related to intercultural health and Indigenous autonomy. Theoretical framework. The study addresses concepts related to Indigenous health, interculturality, medical pluralism, and decolonization of health systems, understanding interculturality as a political and epistemological process. Results. Findings revealed that the three models share a holistic conception of health based on territory, spirituality, and community. SISPI prioritizes self-governance and ancestral medicine; SAFCI incorporates interculturality within the Bolivian state health reform; and FNHA represents a consolidated model of Indigenous health governance. Furthermore, Canada shows higher levels of institutional consolidation due to its structural capacities as a developed country, while Colombia and Bolivia face territorial, financial, and institutional limitations. Conclusions. The analyzed models constitute relevant experiences of autonomy and intercultural transformation of public health policies.
