Antimicrobial Resistance in the Post-Antibiotic Era: Molecular Mechanisms, Emerging Therapeutics, and Evidence-Based Stewardship Implementation in Latin America

Objective: To comprehensively examine molecular mechanisms of antimicrobial resistance, emerging therapeutic strategies, and evidence-based preventive approaches, with particular emphasis on the epidemiological situation in Latin America and Colombia. Method: A narrative literature review was conducted of scientific publications from 2019-2025, using PubMed, SciELO, and specialized repositories. Studies on resistance mechanisms, novel antimicrobial therapies, stewardship programs, epidemiological surveillance, and Latin America-specific data were included. The search incorporated MeSH terms related to antimicrobial resistance, β-lactamases, antimicrobial stewardship, and innovative therapies. Results: Resistance mechanisms include enzymatic modification, molecular target alteration, efflux pumps, and biofilm formation. Novel β-lactam/β-lactamase inhibitor combinations demonstrated efficacy against multidrug-resistant pathogens: cefepime/enmetazobactam for ESBL-producing P. aeruginosa and aztreonam/avibactam for carbapenem-resistant Enterobacterales.. Conclusion. Artificial intelligence accelerated discovery with compounds like halicin and abaucin.
